One sketched out in photoshop then painter and more photoshop, layers and layers!! I have done a few "famous" portraits, and to be honest, i think they are a little too photo realistic on reflection, it's easy to get carried away with what these creative programs can do, sometimes it's better to hold back, that is just as hard to learn as the programs themselves.
